CLEARLAKE, Calif. – Adventist Health Clear Lake has been awarded $6.3 million in PATH CITED 2 funding from the California Department of Health Care Services to expand its Hope Center facility and strengthen rural community health infrastructure across Lake County.
The funding represents a significant strategic investment in addressing homelessness through coordinated health care, housing navigation and supportive services.
The expansion will include construction of space for 29 additional beds, increasing capacity to 50 beds.
The project also includes relocating Restoration House — originally opened in 2017 — to the Hope Center campus, allowing for consolidation of staff, operations, and residents to one site.
“Hope Center has been a shining example of what collaborative relationships can help create,” said Lake County District 2 Supervisor Bruno Sabatier. “City, county, health care, nonprofits and more working all together to help make this be a reality. Even more important is the program itself where the Hope Center team not only provides hope to the unsheltered, but also provides their guests a pathway to a better life with greater independence within our communities.”
Sabatier added, “I have seen the work they do firsthand and have gotten to know the folks working through their program. I've seen the results. Knowing that Adventist Health is anticipating an expansion of the Hope Center philosophy and program gives me greater hope for the future of services to those still unsheltered throughout Lake County.”
“At its core, Hope Center is about dignity, stability and restoring a sense of possibility,” said Ronni Duncan, care management manager at Adventist Health Clear Lake. “Every day, our team walks alongside individuals who are working toward recovery and independence. This expansion allows us to serve more people in a medically appropriate environment while strengthening the support systems that help them transition into permanent housing. We see firsthand how coordinated care, housing navigation and community partnerships change lives — and this investment ensures we can continue building pathways forward for those who need it most.”
Hope Center originally opened in 2020 under the leadership of the nonprofit Hope Rising. In 2023, operations transitioned to Adventist Health Clear Lake, strengthening clinical integration and long-term sustainability.
Since its opening, Hope Center has served 352 individuals, with 121 individuals successfully transitioning into permanent housing.
Current services at the Hope Center include:
• Housing navigation.
• Medical recuperative care.
• Housing tenancy services.
• Therapeutic groups.
• NA/AA support.
• Employment support.
• Meals.
• Medical case management.
• Blue Zones-certified site programming.
With the PATH CITED 2 funding, Hope Center will expand its services to include:
• Medically tailored meals.
• Day habilitation services.
• Provider-ordered community health worker services.
Hope Center plays a vital role in reducing emergency department utilization, decreasing preventable inpatient stays and improving safe discharge pathways for unhoused patients.
By providing a stable, medically appropriate recovery environment, the program helps prevent individuals from being discharged back into homelessness.
The expansion further strengthens cross-system coordination, including collaboration with Probation and other community partners to support stabilization and reentry services.
Adventist Health said the new funding investment marks “a critical step forward” in Lake County’s collective efforts to address homelessness through integrated health care, housing and supportive services — offering not just temporary shelter, but a structured pathway toward stability and independence.
Community members are invited to learn more about the Hope Center expansion and share input at a community input forum on Wednesday, March 11, from 5:30 p.m. to 6:30 p.m.
The meeting will be held at the Hope Center, located at 3400 Emerson Street in Clearlake.
Attendees will have the opportunity to hear about the expansion plans, ask questions and provide feedback as Adventist Health Clear Lake works collaboratively with local partners to strengthen services for individuals experiencing homelessness throughout Lake County.
