NOTICE IS HEREBY FURTHER GIVEN that the Community Development Director will approve this MINOR USE PERMIT with no public hearing if no written request for a public hearing is submitted to the Community Development Department by 4:00 P.M., March 19, 2009. NOTICE OF LAND USE APPLICATION PUBLIC HEARING
AND INTENT TO ADOPT A MITIGATED NEGATIVE DECLARATION

PROJECT TITLE: Lake County Tribal Health
PROJECT LOCATION: 925 Bevins Court APN: 025-462-23
PROJECT DESCRIPTION: Architectural & Design Review and Environmental Review to allow the construction of a 19,680 SF 2-story addition to an existing outpatient medical clinic, interior renovations to the existing 6,151 SF clinic, and related site improvements. Deviation of off-street parking is also requested.
The project is not listed on the Hazardous Waste and Substances Sites List as set forth in Government Code Section 65962.5.
FINDINGS / DETERMINATION: The City has reviewed and considered the proposed project and has determined that the project will not have a significant effect on the environment, with substantial supporting evidence provided in the Initial Study. The City hereby prepares and proposes to adopt a Mitigated Negative Declaration for this project.
PUBLIC REVIEW PERIOD: A 20-day public review period for the Mitigated Negative Declaration will commence on March 6, 2009, and end on March 25, 2009, for interested and concerned individuals and public agencies to submit written comments on the document. Any written comments on the Mitigated Negative Declaration must be received within the public review period. Copies of the Mitigated Negative Declaration are available for review at City Hall, 225 Park Street, Lakeport.
PUBLIC HEARING / MEETING: The Planning Commission of the City of Lakeport will hold a public hearing to consider these applications, the Initial Study, and the adoption of a Mitigated Negative Declaration of environmental impact on Wednesday, March 25, 2009, at 6:00 p.m., in the Council Chambers of City Hall, located at 225 Park Street, at which time and place all interested persons may appear and be heard. NOTICE OF PUBLIC HEARING
NOTICE IS HEREBY GIVEN that the Planning Commission of the County of Lake, State of California will hold a public hearing on application MRS 08-03, GPAP 06-09, RZ 06-13 UP 08-07 and DV 08-05 proposing a Merger and Resubdivision of two lots to create four individual parcels with gross lot sizes being as follows: Parcel 1, 24,000 square feet; Parcel 2, 12,933 square feet; Parcel 3, 21,278 square feet and Parcel 4, 31,522 square feet, a General Plan Amendment from Commercial Resort to Suburban Residential, a rezone from CR-FF, Commercial Resort-Floodway fringe to R1-SC-FF B-Frozen, Single Family Residential-Scenic Combining-Floodway Fringe District, a Major Use Permit to exceed the height limit for residences located in the Scenic Combining District and the creation of a subdivision with three or more lots within the Floodway Fringe Combining District and a Deviation from established road standards to allow for the placement of a gate across a public right-of-way and the construction of a public road beyond a locked gate on 2.06 acres. The project is located at 11967 and 12037 East State Highway 20, Clearlake Oaks, and further described as APNs 035-092-01 and 035-284-10. The project applicant is Benedick Lawson. The Planning Commission will consider adoption of a Mitigated Negative Declaration for this project based on Initial Study IS 07-66.
